Citation text line 29 CFR 1926.404(b)(1)(iii)(C): The employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. a) For employees working at 4753 Chatterton Way in Riverview, as observed on or about December 24, 2014. Employees were using a power saw to cut plywood sheathing that was not inspected for defects prior to use and removed from service. Cord of saw was repaired and missing approximately 12 inches of outer insulation.

The eave to ground height was approximately 19 feet 4 inches and the employees had no fall protection system in place. b) For employees working at 4753 Chatterton Way in Riverview, as observed on or about December 24, 2014. Employees were installing framing lumber on the front overhang with a 5:12 pitch. The eave to ground height was approximately 9 feet 8 1/2 inches and the employees had no fall protection system in place.
