Employees were using a power saw to cut plywood sheathing that was not inspected for defects prior to use and removed from service. Cord of saw was repaired and missing approximately 12 inches of outer insulation. |

The eave to ground height was approximately 9 feet 8 1/2 inches and the employees had no fall protection system in place. |
