A front loader operator, while moving material from the base of the stockpile, suffered asphyxia from material that engulfed the inside cab area, due to a steepened face collapse. The operator had not been provided training on the handling of stockpile material. b) For the front loader operators removing material from the sulfur stockpile on 01/15/2016, at 5740 Pembroke Road, Gibsonton, FL. A front loader operator, while moving material from the base of the stockpile, suffered asphyxia from material that engulfed the inside cab area, due to a steepened face collapse. Safe techniques and/or methods had not been developed and/or established for the safe stockpiling of sulfur material.
