The trench measured 4 feet 7 inches deep, with vertical walls, in class C soil, there were cracks and fissures along the east trench wall and there was standing water in the trench. The spoil pile was placed within 7 inches of the edge of the trench and a safe means of egress was not provided. |

The trench was not inspected by a competent person to identify possible cave-in hazards and to ensure that adequate protective systems were in place prior to permitting employees to perform work within the trench. The trench measured 4 feet 7 inches deep, 3 feet 3 inches wide (side to side), the walls of the trench were vertical walls, the soil was class C, there were cracks and fissures along the east wall of the trench and there was standing water in the trench where the exposed employees were standing. |
