29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about August 27, 2020 and days prior, at the slider, vortex, entry door and main frame departments, employees operated pneumatic die presses without one or more methods of machine guarding to protect them from the moving parts of the die, exposing to crush-by hazards. b) On or about August 25, 2020 and at times prior, at the entry door department, employees operated Drill Presses such as Central Machinery (Item #43389) and RF-31 (Model: 106-1800 and S/N:4333B4) without a guard to protect from the rotating action of the chuck, exposing to caught-in hazards. c) On or about August 25, 2020 and at times prior, at the slider department, employees operated Drill Presses such as Rexon (Model: RDM-100A and S/N: 176523), Delta (Model: 17-965 and S/N: 032160W5081) without a guard to protect from the rotating action of the chuck, exposing to caught-in hazards. d) On or about August 25, 2020 and at times prior, at the vortex department, employees operated Drill Presses such as Delta, Central Machinery (S/N: 030705Q014) and Craftsman (Model: 137.219000 and S/N: RNX1230) without a guard to protect from the rotating action of the chuck, exposing to caught-in hazards. e) On or about August 25, 2020 and at times prior, at the acrylic departments, employees operated Drill Presses such as Central Machinery, Palmgren and Ryobi (Model: DP102L and S/N: AZ 1143 26033) without a guard to protect from the rotating action of the chuck, exposing to caught-in hazards. f) On or about August 27, 2020 and at times prior, at the slider door department, employees operated Birmingham Mill (Model: 1642 and S/N: 0831) without a guard to protect from the rotating action of the chuck, exposing to caught-in hazards. g) On or about August 27, 2020 and at times prior, at the main frame department, employees operated Clausing 15 and Enco (Model: 105-1117 and S/N: 0020036) without a guard to protect from the rotating action of the chuck, exposing to caught-in hazards. h) On or about August 27, 2020 and at times prior, at the vortex department, employees operated Greller & Company Two Point Weld Fixtures (Single Stack / F312SL) without one or more methods of machine guarding to protect them from the moving parts of the heating plates and holding in place, exposing to caught-in and crush-by hazards. i) On or about August 25, 2020 and at times prior, the C-45 horizontal saw was not guarded, exposing employees to flying metal chips. |

In that, the finger guards for the blade guard were missing. i) On or about August 27, 2020 and at times prior, at the main frame department, employees were exposed to struck-by hazard when operating Miter Cut-off Saw, without a point of operation guard. In that, the Plexiglas installed around the blade is not interlock to prevent access to the point of operation during operation. The bottom part of the blade is not covered when not in use. j) On or about August 27, 2020 and at times prior, at the slider department, employees were exposed to struck-by hazard when operating Miter Cut-off Saw, without a point of operation guard. In that, the Plexiglas installed around the blade is not interlock to prevent access to the point of operation during operation and the space between the pneumatic cylinders was covered by a box cutout. The bottom part of the blade is not covered when not in use. k) On or about August 25, 2020 and at times prior, at the back area of the fabrication shop, employees were exposed to struck-by hazard when operating C-45 Horizontal Saw, without a point of operation guard was. |
