29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): a) For the construction site at 6503 Hwy 301, Tampa, FL, as observed on or about 06/26/2012, employees of Lennox Concrete Pumping, Inc. were accessing scaffolds that were 12 ft. high and the working level was only 2/5 planked. Employees were exposed to trip/fall hazards associated with the narrow width of the working platform. |

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used: a) For the construction site at 6503 Hwy 301, Tampa, FL, as observed on or about 06/26/2012, employees of Lennox Concrete Pumping, Inc. were accessing tubular welded scaffolds that were 12 ft. high, and an access ladder was not available in the area and being used. Employees had to climb the frames of the scaffolding. |

No guardrails were present and Employees were exposed to 12. ft. fall hazards onto the concrete surface below, debris, equipment, and protruding pipes. |
