Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a. 1285 St Johns Parkway, Saint Johns: On or about April 26, 2022 five employees were exposed to falls up to 21 feet to the ground below while engaged in installing new roof shingles on a 1 story, 4:12 slope commercial structure and were not protected by any means of personal fall protection, guardrails or safety nets. |

Employees performing roofing work are not protected from eye injuries while using pneumatic nail guns. The employer fails to assure effective protections against eye and face injuries as required by the standard, as demonstrated on April 26, 2022, when employees are working on a commercial roof at 1285 St Johns Parkway, St Johns, Florida, without eye protection while using pneumatic nail guns and are exposed to eye injuries from flying debris or nails. The employer fails to assure effective protections against falls as required by the standard, as demonstrated on April 26, 2022, when 5 employees using a ladder to access a commercial roof at 1285 St Johns Parkway, Saint Johns, Florida, are exposed to a 12 feet 8 inch fall hazard to the lower level in that the ladder does not extend 3 feet above the landing.
