On or about Oct 7, 2020, at the jobsite, employees were directed to work in a trench after two competent persons noticed hazards including cracks and fissures along the trench walls, no safe means of egress, no cave-in protection, and the spoil pile was placed directly at the edge of the trench wall. The trench measured 8 feet deep and was in previously disturbed Type C soil. |
