29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) At the R&D department, on or about May 10, 2021, employees were exposed to an amputation hazard, in that, rotating chuck and unused portion of drill bit were unguarded for the Bridgeport Mill press (serial J-70252) used to drill plastic pipe products. b) At the Warehouse department, on or about May 10, 2021, employees were exposed to an amputation hazard, in that, rotating chuck and unused portion of drill bit were unguarded for the electro spindle drill mounted at the CNC table AXYZ 4008 (serial 6740-6630) used to drill plastic pipe products. |

29 CFR 1910.212(a)(5): Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch: a) At the Injection Molding department, on or about 05/10/2021, employees were exposed to amputation hazards, in that, three fans had metal wire mesh with openings bigger than one-half inch to guard the blades. b) At the warehouse next to the Injection Molding department, on or about 05/10/2021, employees were exposed to amputation hazards, in that, two fans had metal wire mesh with openings bigger than one-half inch to guard the blades. c) At the Extrusion 1 building, on or about 05/10/2021, employees were exposed to amputation hazards, in that, one fan had metal wire mesh with openings bigger than one-half inch to guard the blades. d) At the Extrusion 2 building, on or about 05/10/2021, employees were exposed to amputation hazards, in that, one fan had metal wire mesh with openings bigger than one-half inch to guard the blades. |
