Citation text line 29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity). a) On or about 06/30/2018, at 2600 Technology Drive, Orlando, Florida, employees in the process of bricklaying activities were exposed to a 15'8" fall hazard, in that a guardrail system was not installed on the 3rd level of the side brackets/work platforms of the scaffolding system. b) On or about 06/30/2018, at 2600 Technology Drive, Orlando, Florida, employees in the process of bricklaying activities were exposed to a 22 foot fall hazard, in that a guardrail system was not installed on the 4th level of the side brackets/work platforms of the scaffolding system.

The employer was aware of the in-patient hospitalization on Saturday, 06/30/18. The employer did not notify OSHA of the hospitalization.
