Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about October 30, 2014, at the above addressed site, employees were exposed to a fall hazard of approximately 15 feet 10 inches when performing roof work on a low slope roof while not being protected from falling to a lower level.
