Instance 2: On or about January 7, 2016, at the above addressed job site, an employee was laying cement block units on a 3rd floor slab while being attached to a 9' self-retractable lanyard and attached to a 6' cable that was attached to an anchor embedded in the concrete slab increasing his fall distance which potentially increases the arresting force during a fall. |
