Citation text line 29 CFR 1926.501(b)(4): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: On or about March 31, 2016, at the above addressed jobsite, two employees were installing/removing Plexiglas/Lexan skylights from the top of a Porte De Cochere, without the use of a Personal Fall Arrest Systems, Guardrails, or Hatch Cover(s); wherein one of the two employees was therefore exposed to fall hazards of approximately 18 feet, and which also resulted in the second employee falling approximately 18 feet through a Plexiglas skylight, which resulted in the serious injury of the second employee. FIRST REPEAT: Redco Skylight, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR. 1926.501(b)(4), or its equivalent standard, which was contained in OSHA inspection number 1066207, citation number 01, Item number 001, and was affirmed as a final order on 09/29/2015, with respect to a workplace at 2620 N. Atlantic Boulevard, Fort Lauderdale, Florida.
