On or about May 8, 2013, at the above addressed jobsite, an employee was engaged in the installation of air conditioning duct work on a second floor at approximately 10 feet 6 inches above ground level from a step ladder that was positioned approximately 3 feet from a staircase opening with no guardrails, exposing him to a fall hazard.
