The employer fails to assure effective protection against falls as required by the standard, as demonstrated on February 15, 2019, when an employee is working on a residential roof with a 4:12 pitch, located at 1102 Alabama Avenue, Lynn Haven, FL 32444 without fall protection and are exposed to 11-foot 3-inch fall hazards.
