At the site, employees were exposed to possible cave-ins, in that, an excavation 18 feet by 22 feet and 9 feet deep was not inspected by a competent person knowledgeable with trench excavation safety requirements. Employees were allowed to enter an excavation with nearly vertical walls, without a ladder or other means of egress and with the spoil placed right at the edge of the excavation.
