A) At the construction of the Varela Westshore Apartment Complex, an eight tier scaffold used to construct the stairwell had no safe means of access or ladders and employees were exposed to a fall of approximately 55 feet on or about 08/26/2014.

The employee was exposed to a fall hazard of approximately 55 feet on or about 08/26/2014. The Mark Williams Construction Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1), which was contained in OSHA inspection number 952706, citation number 1, item number 2 and was affirmed as a final order on 02/6/2014, with respect to a workplace located at Varela Westshore Apartment Complex in Tampa, Florida.

Spruce St., Tampa, FL, as observed on or about 12/19/2013, employees were working around and exposed to uncapped, protruding rebar: a) Employees were exposed to uncapped rebar extending out of the ground from concrete caissons measured to be 3ft. and 2in. high. b) Next to the new rebar retrieval pile, there were rebar pieces sticking out of the ground approximately 2-3 feet that employees could fall into. c) Where concrete/form work was being done in the excavated section of the jobsite, uncapped rebar approximately 2-3 feet high was present.
