29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: a) On or about February 11, 2014 at the above addressed job site, employees were exposed to a fall hazard while conducting concrete forming work from a deficient fabricated frame scaffolds that lacked guardrails and were not fully planked, that had not been inspected by a competent person. 2 instances: Instance 1: Fabricated frame scaffold two tiers, approximately 10 feet high, 5 feet wide lacking guardrails on side and end and lacked a plank creating about a 9 inch gap. Lower surface of concrete. Instance 2: Fabricated frame scaffold two tiers high, approximately 8 feet high, 5 feet wide that lacked guardrails on side and end and with one and two 9 inch wide planks creating a gap between 40 and 48 inches. Lower surface of concrete. |
