On or about July 1, 2013, the employer had not established a machine specific lockout/tag-out procedure that had been written and implemented for the performance of maintenance or repair on the Feeding Ironers Machine where the unexpected energizing, startup or release of energy sources could occur and cause injury. |

On or about July 1, 2013, above the table, next to a small folding machine in the shop, an extension cord had been plugged into an electrical outlet on the floor and run up above the table was being used as a substitute for permanent wiring to energize the small folding machine, a fan and a small hand tool. b. On or about July 1, 2013, next to the table, close to the North East side of Exit door in the shop, an extension cord had been plugged into an electrical outlet above the machine, and ran down to the floor, where it was used as a substitute for permanent wiring to energize a small press machine. c. On or about July 1, 2013, on the Blanket Machine, an extension cord, which had been plugged into an electrical outlet above and connected to a fan, was used as a substitute for permanent wiring to energize the fan. |
