On or about October 28, 2015 employees were setting roof trusses on a new residential addition and were utilizing a 12 foot aluminum extension ladder to gain access to the top of the CMU Wall. The ladder was not properly set with 3 feet of the ladder side rails extending above the wall top plate which did not present the employees with any means to steady themselves before descending the ladder from the wall top plate. |
