Citation text line OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees when employees were exposed to struck-by and caught-in-between hazards: On or about 10/10/2012, on the above addressed jobsite, employees were exposed to the hazard of being struck-by and caught-in-between collapsing pre-cast structural members due to non-placement of grout on a least one column. Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow and ensure the subcontractors follow: (a) Project Drawing S1.02 Part I Administration, Section 1.4 Reporting; (b) Part II Inspections, Section 2.4 Structural Precast Concrete. NOTE: ABATEMENT DOCUMENTATION AND CERTIFICATION IS REQUIRED FOR THIS ITEM.
