My insurance carrier at the time required pre-authorization for a procedure that was done. I was not advised at the time of treatment and i didn't give written consent for payment by me.. I had thought it was resolved on several occasions as i contacted XXXX and they assured me it was resolved. However i continue to see it on and off my credit report. There is no justification for this bill. It was an insurance issue that the provider did not do what they were supposed to by getting an authorization. Is there also a no balance billing in place also here?

FROM XXXX INSTEAD IT WAS SENT TO A COLLECTION COMPANY MEDICAL BUSINESS CONSULT OF XXXX XXXX XXXX XXXX XXXX XXXX FLORIDA XXXX ( XXXX ) XXXX ADDITIONALLY I AM ALLOWED UNDER HIPPA LAW ( HEALTH INSURANCE PORTABILITY AND ACCOUNTABILITY ACT OF 1996 ) TO PROTECT MY PRIVACY AND MEDICAL RECORDS FROM THRID PARTIES I DID NOT GIVE XXXX XXXX XXXX XXXX AUTHORIZATION FROM HIPPA TO RELEASE MY MEDICL INFORMATION TO A DEBT COLLECTOR. The CRA is showing a collections account for {$130.00}.
I spoke with the company in XX/XX/XXXXbecause information appeared on my credit report that was new. I asked the company to validate the information but they refused I spoke with XXXX XXXX on XX/XX/XXXX and explained that I DID NOT RECEIVE a letter and notice to dispute the account because the collection agency sent information to the wrong address. The collection agency failed to provide validation of the debt and continues to report inaccurate information to the credit bureaus. XXXX XXXX stated that there was another account with my name for {$530.00}. When I asked her where she had sent the notice she refused to state the date and the address that the notice was sent to.

The first time she called, I told her that I needed some time to figure out when I could pay ... I was in the car at the time. I told her that I would call her back the next day, and I completely forgot.
The second and third times, she was pleasant but getting increasing annoyed that I could not give her an answer.
The fourth time she called, I was on a business trip out of state and in meetings all day. She was audibly angry that I was " putting her off '' and she hung up on me. She then called my wife. While on the phone with my wife ( after learning that I was in business meetings out of state ) she informed my wife that she would " call me everyday while I was there and maybe I would then respond ''.
