On or about 12:30 PM on July 08, 2019, at 1880 metro Parkway, Fort Myers, FL 33906, an employee suffered a work related injury and was hospitalized on the same day of the injury. The employer was aware of the in-patient hospitalization in the afternoon of July 08, 2019. The employer notified OSHA on July 18, 2019 at 1:55 PM of the in-patient hospitalization. |
