Citation text line 29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft: a: On the Scrapper Machine, on or about June 5, 2024, and at times prior, the employer is exposing employees to caught in / between and amputation hazards, in that the shaft end was not guarded by non-rotating caps or sleeves; meaning, there was a protruding shaft end that did not present a smooth edge end that projected out 2 1/2 inches from the end of the 1 inch diameter shaft, which had two screws that protruded 1 inch and 3/4 inch.
