On April 22, 2016, on the 3rd floor, the perimeter of the floor did not have a guardrail system and employees were not wearing a harness and lanyard, exposing themselves to a 22 feet fall hazard. b. On April 22, 2016, on the 3rd floor, the two elevator openings did not have a guardrail system and employees were not wearing a harness and lanyard, exposing themselves to a 22 feet fall hazard. c.
