On or about 12/19/2022, in the Fabrication Yard, employees operated a Scotchman Machine Model DO/100/24M, Serial 1052M1097 to bend, cut and punch metal, that was not equipped with a guard at the point of operation. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.
