29 CFR 1910.119(e)(3)(i): The Process Hazard Analysis did not address the hazards of the process: a. The PHAs did not address the hazards of the process to include the following: 1.The ventilation system for the machine room was not designed to exhaust ammonia vapors out of the room and into a safe area. The three wall mounted fans on the north wall were exhausting the vapors into a space between the buildings permitting vapors to travel into areas of the plant such as but not limited to the boiler room. 2. The machine room was not sealed properly in that; the north wall of the machine room did not extend to the ceiling. The wall terminated approximately two to three feet below the room ceiling. Additionally, openings where piping passed through the walls the opening(s) were not sealed. In the event of a release refrigerant could escape into unventilated areas of the facility. |
