I lived in an apartment in NJ and the thermostat was not working properly. We requested on several occasions for someone to fix because our electric bill was running around {$800.00} due to the ancient equipment. They refused to do any type of maintenance which would have been covered under the lease so we had no choice but to give a written 45 days notice. We knew they were going to keep our security deposit. Some time later we received a statement indicating we owed money for things that were needing fixed that were not true. We do not owe anything to this company and I have been disputing this for some time now.

I do n't know these companies or debts, tried to reach out to get information on debt but nothing was sent or verified to me, I disputed with XXXX, XXXX, and XXXX but it comes back as verified but I did n't receive anything, if this debt is valid ( which it should not be ) I did not authorize my personal information to be shared with a third party company I am allowed under the Health Insurance Portability and Accountability Act ( HIPAA ) to protect my privacy and medical records from third parties. I did not give permission to any of my current or prior medical providers to release any of my medical information to a third party. I am aware that the HIPAA does allows the release of limited information about me but anything more is to only be revealed with the patients authorization. Therefore, my request is twofoldvalidation of debt and HIPAA authorization. I am also XXXX and have been receiving XXXX health insurance such as XXXX and XXXX which was presented as form of payment at every doctor appointment so I 'm not sure why there is even a debt to begin with.

I am a XXXX % XXXX veteran. In XX/XX/XXXX I was injured during a tactical confrontation in XXXX XXXX. In XX/XX/XXXX, I was referred from the XXXX XXXX in XXXX XXXX XXXX Florida, by my primary care physician to see the doctor outside of the XXXX XXXX. The specialist was for XXXX XXXX XXXX and XXXX XXXX. The XXXX XXXX contacted the office, they had discussed payment with the outside care provider and they agreed to take me on as a patient. Shortly after my visit to that office, the clerk who worked at their reception desk attempted to collect the fees for their tests directly from me. I had already provided all the pertinent details about payment. It was to be handled directly with the XXXX XXXX, not by myself. As XXXX % XXXX with the VA for combat injuries sustained while XXXX in defense of out great XXXX. The doctors office continued to harass my wife and myself for payment. I had gone to the XXXX XXXX with a copy of their bill and they had agreed to get in touch with the doctors office and handle they payment they so feverishly attempted to collect from me even after all the proper insured and va documents were provided. I had be reassured this debt was not mine to pay by the VA and their patient advocate. Years later and the debt has now been put into collections, it has been damaging my ability to buy a home for my wife and son, my family. It has disabled me from buying a family car, home, rentals and the like. This has been hanging above my head for a couple years now. I attempted directly through equifax credit agency to dispute this debt owed by the VA hospital to no avail. The debt is not mine to pay yet I 've been paying for it for years because my family can not use our credit to acquire a home or a car. The dollar amount is not mine to pay, {$260.00}. I have been retired from military service and receive a fixed income as a XXXX % XXXX veteran. I can not afford to spend the money that is n't even mine to pay. This has been a losing battle no matter what I have tried to get this debt extinguished. Please help me get this shut down. I really need to do better for my family and having this derrogitory mark on my record is the only thing currently holding me back from being the provider and husband I need to be.

This medical company violated my rights under the HIPPA LAW sold and shared my medical history information with various parties. They also shared my medical information with various credit bureaus and they know my medical history. The hospital was not so suppose to bill me because the visit was under a charity program. This charity program pays the hospital visits in full. Moreover, my medical history is being sold and passed around to different creditors.

QUALITY ASSET RECOVERY XXXX : When me my wife were relocating to Florida, we informed our landlord XXXX XXXX as we did not stay in the apartment for the final month of lease to keep our deposit as final rent and they agreed. Unfortunately, the leasing company still sent us to collections. We did not receive anything in the mail or receive any correspondence regarding anything. We moved to Florida to be closer to my XXXX XXXX and we had no choice but to move. We thought all our bills were handled and closed the door to Jersey. They did not keep to their word and did not kept the deposit without any issue.
