On or about October 21, 2015, on the north side of the frame of a residential house, an employee working on the frame was not protected from a 18 feet fall hazard by the use of a personal fall protection system. b. On or about October 21, 2015, on the north side of the frame of a residential house, an employee working on the frame was not protected from a 11 feet 3 inch fall hazard by the use of a personal fall protection system. c. On or about October 21, 2015, on the east side of the frame of a residential house, an employee working on the frame was not protected from a 14 feet 1 inch fall hazard by the use of a personal fall protection system.
