29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use: a) At Renaissance at Washington Ridge, 150 W 14th Street, Lakeland, FL: On or about 10/4/2023, the employer exposed employees to fall hazards and struck-by hazards, in that, employees were lifted approximately 40 feet onto the roof of a building by a rough terrain, telehandler forklift in a field made wooden box which was not designed or approved by the manufacturer for such use. |

29 CFR 1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. a) At Renaissance at Washington Ridge, 150 W 14th Street, Lakeland, FL: On or about 10/4/2023, employees were exposed to fall hazards of approximately 40 feet, in that a 40 foot Louisville portable aluminum extension ladder used to access the roof of a 3 story building was only extended above the landing surface approximately 20 inches and a grasping device was not provided to assist employees in mounting and dismounting the ladder. |
