At the fabrication shop in 76 Otter Slide Road, Eastpoint, Florida, on or about October 30, 2023, the employer exposed employees to amputation hazards, in that the company had not developed a lockout tagout program with specific energy control procedures for the control of hazardous energy for the Schechtl Max 310/CNC bending machine and the Schechtl SMT 310/BV shear, among other machinery with potential to release stored hazardous energy, nor performed employee training on the control of hazardous energy. |
