On the roof of a temporary commercial worksite, located at 1765 Memory Lane, Jacksonville, FL 32210: On or about October 18, 2023, the employer exposed five employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employees, while using pneumatic nail guns to install roofing shingles. 5 Star Roofing Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1591834, citation number 01, item number 001 and was affirmed as a final order on August 19, 2022, with respect to a workplace located at 29 Woodstone Lane, Palm Coast, FL 32135. |
