On a 6:12 pitched roof of a temporary residential worksite, at 3127 Thomas Street, in Jacksonville, Florida 32254: On or about December 8, 2022, the employer exposed four employees to fall hazards ranging from 10 to 15 feet, in that fall protection was not utilized by employees while installing roof shingles. Maxxim Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1355212, citation number 01, item number 001 and was affirmed as a final order on December 4, 2018, with respect to a workplace located at 2128 S. Cranbrook Avenue, Saint Augustine, Florida 32092.
