On or about November 17, 2023, at building 21- unit 4, employees were exposed to 10 feet - 5 inch fall hazards while conducting work activities on a 6/12 pitched roof, in that, fall protection was not used. b. On or about November 17, 2023, at building 16- unit 7, employees were exposed to 22 foot fall hazards while conducting work activities on a 6/12 pitched roof, in that, fall protection was not used. c. On or about November 17, 2023, at building 11 - unit 8, employees were exposed to 22 foot and 10 foot - 9 inch fall hazards while conducting work activities on a 6/12 pitched roof, in that, fall protection was not used.
