On or about June 12, 2021, and at times prior, at the west side of the building, the employer exposed employees to a 32 feet fall hazard, in that an employee performing roofing work on a 9:12 pitch roof was not wearing a personal fall arrest system. b. On or about June 12, 2021, and at times prior, at the west side of the building, the employer exposed employees to a 32 feet fall hazard, in that two employees performing roofing work on a 9:12 pitch roof were not wearing their personal fall arrest system as intended, according to manufacturer's instructions. Employees were not connected to the D-ring at the back, but on other components of their body harness.
