On or about June 13, 2019, at the worksite located at the Preserve Condominiums in Miami Shores, FL 33138, the employer did not ensure the ladder was properly positioned, an employee was exposed to fall hazard of approximately 13 feet 8 inches to a concrete surface below, while performing painting operations was using 24 foot extension ladder that was placed several feet away from the surface onto which the employee was to gain access to the area around the window, the employee stepped off the ladder then onto the roof above the entrance to the condominium. |
