On or about January 17, 2018, side porch area, employees bracing roof trusses were not protected from falling 9.4 feet to the concrete below by a fall protection system. b. On or about January 17, 2018, 2nd floor, employees installing back deck were not protected from falling 9.4 feet to the deck below. c. On or about January 17, 2018, 3rd floor, employees installing back deck were not protected from falling up to 20 feet to the ground below.
