On or about 5/20/2017 in Winter Garden, Florida an employee suffered a work-related injury and was hospitalized for surgery. The employee was walking through the store and fell breaking her left wrist. The employer was informed of the hospitalization on 5/21/2017. The employer notified OSHA on 5/23/2017 at 7:30 a.m. of the in-patient hospitalization. |
