On or about January 11, 2022, between the 2nd and 3rd floor of Building #3, the employer exposed employees to a 10-feet fall hazard, in that employees doing framing work on the 3rd floor of the building used a job-made ladder that had a broken side rail and did not extend 3-ft above the landing surface.
