On or about September 1, 2020, at 377 Stonehurst Parkway, on the 6:12 pitch roof slopes, three roofing employees installing roofing paper were exposed to an 8 feet fall hazard from the eaves in that they were not protected by a fall protection system. b. On or about September 1, 2020, at 383 Stonehurst Parkway, on the 6:12 pitch roof slopes, two roofing employees installing roofing shingles and drip edging metal were exposed to 8 feet and 14 feet fall hazards from the eaves in that they were not protected by a fall protection system. Alton Roofing Services, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1448962, citation number 1, item number 1 and was affirmed as a final order on March 27, 2020, with respect to a workplace located at 9525 Staple Mill Drive, Jacksonville, Florida. |
