29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): The employer fails to assure effective protection against falls as required by the standard, as demonstrated on February 20, 2020, when employees performing reroofing work on the west side roofs of residential townhomes at 7122 Stonelion Circle, Jacksonville, FL 32256, are not protected from over 20 foot fall hazards by the use of personal fall protection systems. Sky High Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1425704, citation number 2, item number 1 and was affirmed as a final order on November 22, 2019, with respect to a workplace located at 19 Reamoor Drive, Palm Coast, FL 32164. Sky High Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1271499, citation number 1, item number 1 and was affirmed as a final order on November 17, 2017, with respect to a workplace located at 3450 Beauclerc Road, Jacksonville, FL 32256. |

(a) on or about August 23, 2019 employees were using an aluminum 20 foot, extension ladder to gain access to the second story roof, however, the ladder was too short to extend 3 feet above the landing surface in accordance to the requirements of this standard. |
