On or about 07/12/2018 at 8455 Adalina Dr Orlando FL, at a single family residential home construction site, an employee was exposed to a 10 foot fall hazard during concrete pumping, in that, a six foot metal step ladder was leaned up against the wall to gain access to the top of the cement block walls without being provided ladder training to include at a minimum the following: 1. The nature of fall hazards in the work area. 2. The correct procedures for erecting, maintaining and disassembling the fall protection systems to be used. 3.
