On or about May 17, 2024, at 12893 SW 232nd Avenue, Miami, Florida, an employee was exposed to a fall hazard of approximately 15 feet to ground level while engaged in plastering work without having training to recognize and avoid the hazards associated with working from scaffolding. |

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: On or about May 17, 2024, at 12893 SW 232nd Avenue, Miami, Florida, employees were exposed to a fall hazard of approximately 15 feet to ground level while engaged in plastering work without fall protection such as a complete guardrail system. All In Stucco LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1)(vii), which was contained in OSHA inspection number 1670572, citation number 1, item number 2, and was affirmed as a final order on 11/17/2023, with respect to a workplace located at 22985 SW 135th Avenue, Miami, Florida 33102. |

Cross braces were used as a means of access: On or about May 12, 2023, at 22985 SW 135th AVENUE, Miami , Florida, employees were exposed to a fall height of approximately 12 feet to ground when accessing a scaffold without a safe means of access/egress. |
