On or about 01/22/2020, at the site, customer delivery employees were exposed to possible cave-ins, while in a trench excavation 4 feet wide, 14 feet long and 5 feet 8 inches deep. The employees were not provided training and education on trench safety and hazard recognition relating to trench conditions such as soil separation, previously disturbed soils and soil classification as well as safety precautions for each soil classification. |

On or about O1/22/2020, at the site, Duke Energy Florida - Customer Delivery employees were exposed to possible cave -ins, in that, inspections were not conducted by a competent person to assure that while employees were in a trench excavation 5 feet 8 inches deep and 4 feet wide by 14 feet long, shoring, sloping or other protective measure were used, a ladder was provided for safe egress and the spoil was placed at a safe distance from the trench walls. |
