On or about April 18, 2016 3 employees were working on the third and fourth tiers of a tubular welded frame scaffold system that was not fully planked between front and back uprights and were exposed to falls in excess of 18 and 24 feet to the ground below. |

Cross braces and End Frames were used as a means of access. a). On or about April 18, 2016 3 employees used the end frames of the scaffold tiers to gain access to the upper tiers of the scaffold. |
