On or about June 21, 2018, at a new construction, single-family, single story residence at the DR Horton homebuilder's Magnolia Manor community in Ocala, Florida, two employees used the top half of a two piece extension ladder that was not extended at least 3-foot above the upper landing surface and exposed the employees to fall hazards.
