Citation text line 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On or about October 30, 2017: an employee was working on a roof with a pitch of 4:12. The employee was throwing old shingles and debris that were removed from the roof to the dumpster while standing on top of the material. The material shifted causing the employee to slipped and fell to ground. The employees was not protected by guardrail system, safety net, or personal fall arrest system, nor was the employee provided with an alternative fall protection measures, the employee fell to ground about 25-feet resulting in fractures. Perry Roofing Contractors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1175589, citation number 1, item number 1 and was affirmed as a final order on October 18, 2016, with respect to a workplace located at 3726 SW 40th Gainesville, FL 32608.
