On or about June 25, 2014, at the top of the stairs in the left duplex unit, employees who accessed the second-floor were exposed to and 11 feet fall hazard in that a guardrail system was not installed on the stairs. b. On or about June 25, 2014, in the roof trusses of the second-story of the left duplex unit, an employee installing bracing was exposed to a 9 feet fall hazard to the floor in that he was not protected by a fall protection system. |
