On or about November 4, 2015, on the second level of the scaffolds at the front side of the house, employees applying stucco cement were exposed to a 13 feet fall hazard in that the scaffolds were not fully planked. b. On or about November 4, 2015, on the second level of the scaffold at the southwest corner of the house, employees applying stucco cement were exposed to a 13 feet fall hazard in that the scaffolds were not fully planked. c. 29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Endframes were used as a means of access: a. On or about November 4, 2015, along the south side of the garage, a means of access was not installed for employees who accessed the second level of the scaffold by climbing the endframes, exposing themselves to a 13 feet fall hazard. |

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section: a. Employees installing stucco at elevations of six feet or more above the ground are not being protected from fall hazards by a fall protection system. The employer fails to assure effective protection against falls as required by the standard, as demonstrated on November 4, 2015, on the 6:12 pitch roof over the garage, an employee placing a protective tarp was exposed to a 10 feet fall hazard from the eave, and on the 6:12 second story roof above the front entrance, an employee placing a protective tarp was exposed to a 20 feet fall hazard from the eave in that they were not protected by any fall protection systems. Orquicely Construction, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1042322, citation number 01, item number 001 and was affirmed as a final order on May 12, 2015, with respect to a workplace located at 23 Forest Edge Dr, Saint Johns, Florida. |
