At 6810 S. Hwy A1A in Melbourne Beach, Florida, on or about June 2, 2021, and at times prior, on the 6:12 pitch roofs of the house and garage, three roofing employees installing sheet metal roofing panels were exposed to 8 feet and 18 feet fall hazards in that they were not protected by a fall protection system. Kuzak Roof Maintenance, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1384742, citation number 1, item number 2 and was affirmed as a final order on September 6, 2019, with respect to a workplace located at 2800 Business Center Boulevard in Melbourne, Florida.
