The crew supervisor and employees performing roof sheathing work on a roof six feet or more above the ground are not protected from fall hazards by a fall protection system. The employer fails to assure effective protection against falls as required by the standard, as demonstrated on October 28, 2016, when employees were attaching the roof sheathing on the 5:12 pitch roof of a commercial store building at 3957 Hendricks Avenue, Jacksonville, Florida 32207 without the use of complete fall protection systems and were exposed to fall hazards of 27 feet and 13 feet 10 inches.
